We provide consistent, compassionate mental health care directly within long-term care facilities. Our experienced mobile teams partner with over 40 nursing homes across Northeast Ohio to ensure residents receive the support they need – right where they live.

At Open Water Counseling & Recovery, we believe every individual deserves high-quality mental health care – regardless of age, setting, or circumstance. This program brings licensed professionals directly to residents in long-term care, offering continuity, dignity, and trusted support.

This level of care is ideal for nursing home residents who:

• Are managing depression, anxiety, or behavioral concerns
• Have psychiatric needs requiring consistent medication oversight
• Would benefit from counseling, case management, or stabilization services
• Are at risk for medication overuse or require gradual dose reduction (GDR)

Our multi-disciplinary care teams provide individual therapy, psychiatric evaluation and medication management, case management, and behavioral support, all delivered onsite. We also offer antipsychotic monitoring, gradual dose reductions when clinically appropriate, and ongoing staff training in de-escalation and non-medication interventions. 

All services are supported with timely documentation and collaborative communication with nursing home staff. At Open Water, our goal is simple: to make mental health care accessible, respectful, and consistent for those who need it most.
